Veterans and Medicare: Could You Lower Your Part B Premium and Receive Extra Benefits?

For Medicare-eligible veterans and dependents with TRICARE For Life or CHAMPVA: how Medicare Advantage plans may lower your Part B premium and add dental, vision, hearing, and fitness benefits.

If you're a Medicare-eligible veteran or dependent

Many veterans and their family members qualify for more than one health coverage program at the same time. If you have Medicare Part A and Part B, you may also be covered by TRICARE For Life. Certain spouses and dependent children may be covered by CHAMPVA. Each program has its own rules about what it pays and when, and understanding how they work together matters before you make any changes to your coverage.

TRICARE For Life in plain terms

TRICARE For Life is TRICARE's coverage for people who have Medicare Part A and Part B. It works alongside Medicare: Medicare pays first for your care, and TRICARE For Life pays second, covering many of the costs Medicare leaves behind, such as deductibles and coinsurance.

One rule is especially important: to keep TRICARE For Life, you must keep both Medicare Part A and Part B. If you drop Part B, TRICARE typically treats you as having no health coverage and your TRICARE coverage ends.

CHAMPVA in plain terms

CHAMPVA is a Department of Veterans Affairs program that helps pay for health care for the spouse or dependent children of certain veterans with permanent, service-connected disabilities, or of veterans who died in the line of duty or from a service-connected condition. The veteran himself or herself is not covered by CHAMPVA; veterans generally use VA health care instead.

CHAMPVA beneficiaries who are also Medicare-eligible are generally required to enroll in Medicare Part B when first eligible. CHAMPVA then pays after Medicare for covered services.

How Medicare Advantage fits in

Medicare Advantage (Part C) plans are an alternative way to receive your Medicare Part A and Part B benefits through a Medicare-approved private insurer. Many plans bundle prescription drug coverage and extra benefits into one plan, often with a low or $0 monthly plan premium.

If you enroll in a Medicare Advantage plan, that plan replaces Original Medicare as your primary coverage, and TRICARE For Life or CHAMPVA coordinates with it differently than with Original Medicare. Network rules and claims procedures may also change. That's why it's worth reviewing coordination with TRICARE For Life or CHAMPVA before you enroll — a licensed agent can walk through your specific situation with you.

The Part B premium reduction (giveback)

Some Medicare Advantage plans offer a Part B premium reduction, sometimes called a giveback benefit. The plan pays part or all of your monthly Medicare Part B premium — money you would otherwise pay to Medicare yourself.

A few things to know:

  • Not every plan offers it, and it's only available in certain counties and plan areas
  • The reduction usually shows up as a lower monthly premium, not a cash payment to you
  • You must remain enrolled in the plan to keep the reduction
  • Veteran status alone does not guarantee eligibility — the benefit depends on the specific plan

Extra benefits some plans may include

  • Routine dental care: cleanings, X-rays, and more
  • Vision benefits: eye exams, glasses, and hearing aid benefits
  • Hearing care: exams and hearing aids through some plans
  • Fitness benefits: gym memberships and fitness programs

When you can make changes

Your Initial Enrollment Period is the seven-month window around your 65th birthday. After that, the Annual Enrollment Period runs from October 15 to December 7, when you can join, switch, or leave a Medicare Advantage plan. People already in a Medicare Advantage plan have a Medicare Advantage Open Enrollment Period from January 1 to March 31. Certain life events may qualify you for a Special Enrollment Period at other times of the year.

Where to learn more

Official sources include Medicare.gov (who pays first, plan costs, and joining a plan), TRICARE.mil (TRICARE For Life and TRICARE with Medicare), and VA.gov (CHAMPVA benefits and the CHAMPVA Guidebook). Benefits vary by plan and location, eligibility and enrollment periods apply, and you must maintain Medicare Parts A and B and continue paying your Part B premium. Low or no plan premium does not mean no out-of-pocket costs.
